Hometown Heroes Nomination Form

In order to qualify for a Hometown Hero award,
  • Contributions must have taken place in the 2025 calendar year.
  • Nominee must be a current resident of Madison County for the entire 2025 calendar year or must be employed in Madison County for the entire 2025 calendar year.
  • Individuals/Businesses/Organizations may be nominated under more than one category but must have a separate nomination form for each.
  • Completed nomination forms must be submitted to Madison Parks and Recreation by Friday, December 5, 2025. Online voting ($2 per vote) will take place through January 15, 2026, with the final recipients honored at our Hometown Heroes Community Awards Gala on February 21, 2026.
  • Award recipients will receive a ticket to the Gala.
