Hometown Heroes Nomination Information

ALL NOMINATIONS ARE DUE BY FRIDAY, DECEMBER 8, 2023
Forms can be downloaded from the file below OR completed via Google Sheets (see below)
In order to qualify for a Hometown Hero award,
  • Contributions must have taken place in the 2023 calendar year.
  • Nominee must be a current resident of Madison County for the entire 2023 calendar year or must be employed in Madison County for the entire 2023 calendar year.
  • Individuals/Businesses/Organizations may be nominated under more than one category, but must have a separate nomination form for each.
  • Completed nomination forms must be submitted to Madison Parks and Recreation by Friday, December 8, 2023. Online voting ($2 per vote) will take place through January 15, 2024, with the final recipients honored at our Hometown Heroes Community Awards Gala on February 17, 2024.
  • Award recipients will receive a ticket to the Gala.

Educator of the Year

This award is dedicated to honoring the tireless efforts, commitment, and innovative initiatives of a non-profit that has made a significant difference in the lives of those it serves and the broader community it impacts.

Youth Impact Award
A prestigious recognition presented to an individual who has demonstrated exceptional dedication, commitment, and selfless contributions to the betterment of young people within their community or organization. This accolade acknowledges the recipient's tireless efforts, positive impact, and unwavering support in nurturing and empowering the next generation, making them a source of inspiration and role model for others.

Community Service Award
A general award that recognizes individuals who have made significant contributions to the community's growth through their volunteer work and in-kind support.

Personal or Corporate Philanthropy Award
Recognize local businesses or corporations that have demonstrated a strong commitment to community growth through their financial contributions and employee volunteer programs.
 
Environmental Stewardship Award
For those who have dedicated their time and resources to environmental initiatives that contribute to the sustainability and growth of the community.

Community Hero Award
Highlight unsung heroes in the community who have made a significant impact on community growth, often behind the scenes.

Cultural Enrichment Award
Recognize individuals or groups that have contributed to the cultural diversity and enrichment of the community through their volunteer work and support of cultural events.

Educational Excellence Award
Recognize those who have volunteered to improve educational programs and opportunities in the community, such as tutoring or mentoring.

Mentorship Excellence Award
Celebrate individuals who have made a difference by providing guidance, mentorship, and support to others in the community, helping them achieve their goals.

Community Patriot Award
This award is designed to celebrate and honor the dedication and sacrifice of those who have served or are currently serving in the military within your community. It not only recognizes their commitment to the nation but also acknowledges their ongoing role in building a strong and resilient community.
